We handle personal information, including personal health information, in accordance with the privacy laws that apply to our practice in Ontario and Canada, including Ontario’s Personal Health Information Protection Act (PHIPA).
Information we collect through this website
- Information you choose to send through our forms: your name, email address, phone number, the treatment you are interested in and your message.
- Basic technical information that any website receives, such as your IP address, browser type and the pages you visit, used to keep the site secure and working properly.
Please do not include detailed medical history in a website form. We will gather the health information we need in person, during your private facial and skin assessment.
Information we collect in our practice
When you become a client, we collect the personal health information needed to assess you, plan and provide your treatment safely, and keep the clinical records we are required to keep. This may include your contact details, health history, medications, allergies, treatment notes, consent forms and photographs taken for your clinical record. Clinical photographs are never used for marketing or shared without your separate, written consent.
How we use your information
- To respond to your request and arrange your appointment.
- To provide, document and follow up on your care.
- To meet our professional, legal and regulatory obligations.
- To send you information about Maison 58 only if you have given us your express consent, which you can withdraw at any time.
We do not sell, rent or trade your personal information.
Who we share it with
Your information is shared only with those who need it to provide your care or operate the practice, such as our medical director, Alison Anger, NP, and service providers who host our website and email under confidentiality obligations. We disclose information without your consent only where the law permits or requires it.
